Self-Referral for Patients
The Cancer Society has a range of FREE services available to adults affected by cancer. These services are designed to provide support for patients and their families/supporters through diagnosis, treatment and preparing for life afterwards.
The Cancer Society's services requiring a referral include:
- Driving service - Patients are collected from their homes and taken to and from treatment appointments
- Community Nursing service - Cancer Support Nurse Specialists visit patients in their homes providing practical information and support to help people manage throughout treatment
- Psychological support - Psychology service (Auckland) and counselling (Northland) provide patients and their loved ones strategies for coping with cancer
To access the services provided by the Cancer Society, either complete a self-referral form (below), phone the Cancer Society information line (0800 CANCER (226 237)) or ask your health care professional to refer you.
